Chelsea sealed the title with victory over Crystal Palace on Sunday as Eden Hazard tucked in the rebound after seeing his penalty saved by Julian Speroni in the first half at Stamford Bridge.

The Belgian's header proved decisive despite a determined display from Alan Pardew's visiting squad. The 1-0 victory means Chelsea have 83 points from 35 games and cannot be surpassed by closest rivals Manchester City, who visit Tottenham Hotspur later on Sunday, and Arsenal both of whom trail by 16 points.

The title win sparked a massive response on social media as fans celebrated a first Premier League trophy at Stamford Bridge since 2010. But it was the official Twitter account of Chelsea which was causing the biggest stir.

The club's media team altered the club name to simply read: "CHAMPIONS!"

But the players will have to wait to lift the silverware. Premier League tradition stipulates that the champions be awarded their trophy on completion of their final home fixture of the season.

In Chelsea’s case, that falls on the final day of the season itself: their home match with Sunderland on Sunday May 24.
